Works with patients, families, physicians, other disciplines, other health service providers, and insurance companies to facilitate patient movement through the continuum of care, and provision of care that is in the best interest of the patient. Managers cases to ensure timeliness of service, appropriateness of patient placement according to level of service needs, cost-effectiveness, achievement of the highest practical level of patient and family satisfaction achievement of quality patient health outcomes, compliance with legal, regulatory, and accreditation requirements, and appropriate utilization and reimbursement of services. Functions as a resource within the organization and the community it serves. Provides clinical leadership within the organization for the implementation of the organizational utilization and outcome management programs. Graduate of a nursing diploma program, associate's degree in nursing program, or bachelor's degree in nursing program from an approved school of nursing required, BSN preferred. Minimum 2 year recent clinical RN acute care experience and 2 years recent case management experience. Utilization review knowledge a must. Current and valid PA Registered Nurse license required.
